DJO Global billed me for a medical device for which I have already paid the provider, Verde VAlley Medical Center, Northern Arizona Healthcare. The date of service on the bill is incorrect by several weeks. DJO Global claims, fraudulently, that I signed “electronically or on paper”” a “”DJO Global Patient Product Agreement Form.”” I find it curious that the bill came from an address in Pennsylvania
